The Daily PANCE and PANRE Question 46
A 17-year-old male is stabbed in the anterior chest with an ice pick. He quickly becomes hypotensive, tachycardic and obtunded. His breath sounds are equal, his neck veins are distended, and his chest x-ray is normal. What condition is most likely? Simple pneumothorax Tension pneumothorax Pericardial Tamponade aortic laceration distal to the origin of the left common carotid artery inferior vena cava laceration Answer:C Pericardial Tamponade This is the typical clinical presentation of tamponade (hypotension, JVD). He was stabbed and probably has a mediastinal bleed (so, I think the mediastinum would be widened on his chest film but the "normal CXR" is what rules out the pneumothorax - another common problem in a stabbing injury. I take my boards tomorrow and was a mess until à I reviewed your exams.à The humor was great and really helped to ease my tension.à Keep up the great work! Marilyn OteroPA-C Good morning. I purchased a three-month trial of SMARTYPANCE with the hopes of using it as an adjunct to other study materials that I had purchased. Almost immediately, I ditched everything else and used your website exclusively. I work full time and I have two little ones at home, so just like everyone else, it was hard for me to manage my time. My certification is ending this year and I timed it so that if I didnââ¬â¢t pass, Iââ¬â¢d have another opportunity to re-take it in December if I failed. I just found out this morning that I passed my PANRE! I studied for one {stressful} month, and though I wouldnââ¬â¢t advise cramming everything in such a short period of time, your website really helped me so much. Thank you for putting all of this content out there for us. You are amazing! Jenssen Parungao, PA-C Stephen, I want to thank you for creating the Podcast and of course the academy website. I just took my Panre for the 2nd time and I passed. I cant thank you enough for your creativity, humor, and innovation when creating such great tools for all of us. I have been a PA for 13 years and I was so worried about the PANRE given that now I have 2 small kids, have a coaching business, and work full time as a PA as well. I listened to your podcasts daily for 3 months. I did your tests on the academy and I am so grateful for how refreshing your programs are. If you ever need a testimonial somewhere else please contact me. Of course I will recommend your program to everyone. Big hug and big thanks from a happy fellow PA. Sincerely, gina Gina, PA-C
